Registration

Registration is closed for this meeting.  However, we can still add you on but there is a late registration fee of $30 in addition to the regular fees.


All courses will be submitted to ARBO for COPE approval and to both the Texas and South Carolina boards of optometry

HOW TO REGISTER

We have 3 ways to register for our meeting

  1. Download the registration form below in either Word or PDF, print it, and mail to us with a check
  2. Download the registration form below and fax to us or send as an email attachment.  We will then send you a link to pay by credit card.
  3. OUR NEWEST OPTION: REGISTER AND PAY ONLINE.  


If you are using a different card than attendee please write attendee name in notes with OE tracker number


CANCELLATION POLICY: All cancellations are subject to a $50 fee. No refund or credit is given after February 15th or for no-shows. 

NO EXCEPTIONS. By registering for the course you are agreeing to these terms.


 CONFIRMATION will be EMAILED to you after payment has been processed. 

We communicate almost exclusively by email so make sure that our email is on your safe list so our emails don’t go into your spam folder. 

All courses are COPE approved. It is your responsibility to see if your state accepts these for CE credit
